Monthly Marketing Update- November 2025

Last month we celebrated Case Management Week by coordinating the delivery of goodies and thank you notes to 27 hospitals from Medford, OR up to Everett, WA. More than 200 providers participated to show their appreciation for case managers. We continue to hear from hospital staff how much the recognition meant to their teams. Our team had communicated to providers that these were intended as “no contact” drop-offs, with limited or no face time expected. Ironically, that very approach is what touched case management leadership the most- they were deeply moved that so many providers took the time to say thank you with no expectation of anything in return.

The “Transitions of Care” meeting’s goal is to improve communication between case management and community providers, especially related to readmissions and start of care following a hospital visit.
