Monthly Marketing Update- August 2025

We hope you're enjoying a fun and relaxing summer! As we roll into August, we’ve got a few important updates to keep on your radar.

First up: our next Transitions of Care Meeting is tomorrow-Tuesday, August 5th at 11:30 AM. This bi-monthly call brings together providers and case managers from across Oregon and Washington to strengthen collaboration around hospital discharges, readmissions, and start of care coordination.
This month’s agenda includes:
  • Continued planning for Case Management Week in October
  • Exploring strategies to improve communication with non-skilled care settings

We also have a busy month ahead with two upcoming Case Management events in Southern Oregon:
  • Asante Three Rivers – Wednesday, August 20th
  • Providence Medford – Thursday, August 21st

We are honored to be the official resource for the Alzheimer’s Association, as well as several hospitals and Area Agencies on Aging.
