Monthly Marketing Update- August 2025
We hope you're enjoying a fun and relaxing summer! As we roll into August, we’ve got a few important updates to keep on your radar.
First up: our next Transitions of Care Meeting is tomorrow- Tuesday, August 5th at 11:30 AM. This bi-monthly call brings together providers and case managers from across Oregon and Washington to strengthen collaboration around hospital discharges, readmissions, and start of care coordination.
This month’s agenda includes:
- Continued planning for Case Management Week in October
- Exploring strategies to improve communication with non-skilled care settings
Registration is full for the upcoming EvergreenHealth Case Management Fair on September 30th at 11:30 AM. We are working on a second date in April for our sponsors.
